Patients with this status are receiving advanced life support therapies and/or living full-‐time in intensive care units until donor hearts become available. 1A patients are the first to receive available hearts. 1B is the second most urgent status classification on the wait list. Patients with 1B status are next in line behind 1A patients when donor hearts become available. While awaiting hearts, 1B patients require intravenous medications called inotropes in order to keep their hearts functioning. Depending on their condition, 1B patients can either live at home or in a hospital.

- Life expectancy of a week or less without a transplant (1A)
- Require mechanical heart support device in-‐hospital (1A)
- Under the age of 18, pending other status requirements met (1A/1B)
- Require continuous IV meds and mechanical heart support device in-‐home (1B)
